You can now add Addendums to completed notes in Charting +! This function is available on all note types. In order to add an Addendum, open your already completed note and scroll to the bottom. (For Assessment Notes, you'll need to select the Expand Treatment Information tab)
Select the + button to open up the Addendum section:
The date will always pre-populate to the date the addendum is made and the provider name will also populate to whoever is logged in.
When you create the Addendum, you can select a related field that corresponds with what is said in the Addendum but this is not a required field and can be left at NONE. If you do choose a field, the text you choose will be carried over into the Initial Assessment Report, Progress Report and Discharge Report for the following fields only:
Assessment
Clinical Impression -> Clinical Impression
Manual Therapy, Therapeutic Exercises, Therapeutic Modalities, Education -> Proposed Management Plan
Additional Notes -> Summary
Re-Assessment
Clinical Impression -> Clinical Impression
Manual Therapy, Therapeutic Exercises, Therapeutic Modalities, Education -> Proposed Management Plan
Level of Progress -> Level of Progress
Treatment Plan -> Treatment Plan and Summary
Describe any factors that may impede recovery -> Barriers to Recovery
Discharge
Clinical Impression -> Clinical Impression
Treatment Provided -> Treatment Provided
Reason for Discharge -> Reason for Discharge
Discharge Plan -> Discharge Plan
Discharge Summary -> Discharge Summary
You also have the ability to add multiple addendums to a patient profile - you can do this all at once or later on down the road as the + button will always be there.
Once you're finished, you will select Complete Note to save. After you select Yes on the confirmation message, this is what your completed Addendum will look like:
If you go back to the patient profile, you'll notice an asterisk beside the completed note. This symbolizes that there is an addendum attached to the note:
