Block Billing (OCF-23)

All claims submitted as part of a MIG Block billing must be a combined charge. Even if you are only submitting a single procedure code on a claim you must still combine under the codes in bold below.

To combine a charge do the following:

• Open Client Folder
• Click Records Tab
• Select Treatment Plan
• Click Procedures Tab
• Highlight all procedures you want to combine
• Right-click
• Click Combine Charge
• Select Block code to combine under (Codes Bolded below, price is listed)
• Click OK

• 2ZZ02 - Total Body Assessment
• MIG01 - Block 1 (Weeks 1 to 4) ($775)
• 1ZX12 - Therapy (Multiple Body Sites)
• MIG02 - Block 2 (Weeks 5 to 8) ($500)
• 1ZX12 - Therapy (Multiple Body Sites)
• MIG03 - Block 3 (Weeks 9 to 12) ($225)
• 1ZX12 - Therapy (Multiple Body Sites)
• MIG24 - Discharge Report ($85)
• 7SJ30 - Documentation Support Activity
• 7SJ30LB - Documentation Support Activity for Claim Form
• MIGSG - Supplementary Goods and Services (Up to $400)
• GXX01 - Back Roll
• GXX08 - Collar
• GXX07 - Cane
• GXX09 - Crutches

Any other documentation to be billed must be exempt from the payment plan when you are creating your claim:
