PDF Forms

Once the assessment is completed, the system will automatically recommend some forms you may need to fill out based on what you have selected. The forms will pull the patient and provider information from the note that you completed

• There are 3 sections now: Default Forms, Government Forms and Send to InnoCare Software.
• Choose Download PDF for the form you would like to complete
• If you do not see the form that you need in the list, choose “Add another form”
